This achievement requires you to activate all remote links, battle/defeat/purchase all villains found by said remote links. So if you're missing a villain or if they're not appearing try visiting their nearby terminal again and interacting with it to get them to spawn. Maybe the game glitched and forgot to load him onto the map and I just had to interact with the terminal again. It's just weird because I definitely had the entire Gotham map unlocked so I don't know why his head icon didn't appear while viewing the map.
Either way, he popped up and I defeated him to get the achievement. I didn't recall ever accessing that terminal but it didn't have a red light pillar like the other terminals (that were previously unlocked) which means you haven't accessed it yet. I had to actually go up to the Harbourside Theatre terminal and access it for him to show up (again?). This was odd to me because the entire map of Gotham had been unlocked which means all the villain head icons should have appeared on it so I thought it was glitched. I opened up the selectable Free Play characters and noticed there was still a silhouette for Mad Hatter. After accessing all the terminals I used the map of Gotham and followed all the villain head icons then defeated and purchased each one but the achievement didn't unlock.
